Volunteer Forms

If you would like to volunteer with The Arc of San Diego on an ongoing basis, please fill out the following documents:

• The Volunteer Application
• Volunteer Release and Waiver of Liability
• Criminal Record Check
• Volunteer Drug and Alcohol Screening Consent Form
• Picture and Name Release

 If you will be volunteering with The Arc of San Diego for a one-time, hands-on project, please fill out the following documents:

• Volunteer Release and Waiver of Liability
• Minor Waiver of Liability (if participant is under the age of 18)
• Picture and Name Release
